Home Bound Outreach Services Request Form

Homebound Outreach services can be provided to any Jefferson County resident who is unable to get to a library branch for more than 2 consecutive months. This can be due to transportation issues, disability or any extenuating circumstance subject to library staff discretion. If you or someone you know is interested in Homebound Outreach Services please fill out the form below or call 812-265-2744. All library materials are pre-selected, checked out, delivered and picked up on pre-scheduled days, once a month, according to a schedule convenient for patrons and staff.
